Our Covid Waiver Form

Please complete this form 24 hours before each & every atttendance at the dance studio

The NHS states the main symptoms of coronavirus are:

  • a high temperature – this means you feel hot to touch on your chest or back (above 37.5)

  • a new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual)

  • a loss or change to your sense of smell or taste – this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal

 

Most people with coronavirus have at least 1 of these symptoms.

Further Information about this waiver

  • Personal details may have to be provided to the Vault and NI NHS in the event of a Covid19 outbreak for track & trace purposes – Your information will not be released without your permission unless under these circumstances

  • Should you present with Covid19 symptoms you should inform us immediately so that we can implement our emergency response protocols

  • Before entering the building we will take your temperature with a no-contact thermometer - anyone with a temperature over 37.5 will be refused entry

  • You will be asked to wear your mask throughout the building - only removing this once the class is ready to start - masks must be put back on at the end of the class

  • You will be asked to use the no-touch automatic alcohol rub dispensers upon entry to either dance space, and again after the class & before leaving the dance space (if you are allergic to these then you will need to supply your own anti-allergy hand sanitiser)
